As you and your loved ones make decisions about your health care, thinking about and specifying the things that are important in your life can help guide your care team towards the care you want today and in the future. Advance care planning helps you understand treatment options and provides you a voice in your care decisions. Making these important decisions now can allow you and your loved ones to focus on living well in the future. You’ll have peace of mind knowing your care team is aware of the treatment options you choose, and that your loved ones will not have to make these important decisions for you.

  • Are my goals for medical interventions compatible with my personal goals, values and religious beliefs?
  • Have I made my preferences clear to my doctor and loved ones?
  • Have I put my wishes directives in writing?
  • Have I chosen a health care agent?

We encourage all of our patients to have a conversation with their loved ones and write down their thoughts in a legal document, called a Health Care Directive, so that we better understand your care goals and wishes.
