A referral source for people with various issues – Common needs they refer for are help with rent, utilities, etc.
SENIOR LINKAGE LINE: 612-333-2433
An information and referral line for seniors and/or disabled people. Provides referrals for such things as Meals on Wheels, homemaker services, transportation, home health care, etc.
METRO MOBILITY: 651-602-1111
Assists patients with transportation and from appointments. Eligibility for this program requires the patient to complete and mail in the Americans with Disabilities Act application. Fees for rides range from $2.00 to $2.50 each way. Wheel chair accessible
SOCIAL SECURITY ADMINISTRATION: 1-800-772-1213
Call to apply for Social Security disability, supplemental security income, and for information on available programs. When you call this number an operator will schedule an appointment to determine specific programs for which you may be eligible. www.ssa.gov
AMERICAN CANCER SOCIETY: 1-800-ACS-2345
This national number will supply patients and families with a local number. The local number will put you in touch with a navigator who is an expert on ACS services. Common services include: transportation, home medical equipment (provided free of charge) and referrals to many support groups.
CANCER SURVIVORS NETWORK: 1-877-333-4673
(HOPE)
A 24-hour hotline for patients and caregivers, as a service of the American Cancer Society. Provides recorded discussions from cancer survivors on a variety of topics.

Cost is $5.00 and requires a physician’s signature.
You can also ask your clinic nurse for an application form.
MINNESOTA HOSPICE ORGANIZATION: 651-659-0423
Provides referrals, educates caregivers and does hospice training for the public
